This never-used spiral bound purple ridged cardboard covered address book is by Jody Bergsma (© 2000 Jody Bergsma Galleries, Inc.).
At the back of the book is a monthly section for birthdays, anniversaries, etc.
Something had pressed on the book while it was stored, and the front cover has some indentations on the elevated ridges.
Note Worthy Addresses Bergsma is in silver-tone lettering on the front.
